A U.S. Pacific Fleet Integrated Undersea Surveillance Systems officer\, Brian died while on an off-duty closed-circuit rebreather (CCR) training dive in Honolulu. \nWindal recently contacted Scubadillos\, asking to be a speaker and share the elements of creating a Just Culture to prevent such tragedies. **Meeting details**Our meeting begins **at 7 p.m**.
